Billing FAQs

What is your financial policy?

As your physician(s), we are committed to giving you the best possible medical care. To achieve this goal, we need your assistance and understanding of our payment policy.

  1. It is your responsibility to provide us with your most current insurance information. If you fail to provide accurate insurance information in a timely manner, your insurance company may deny the claim. If the claim is denied, you will be financially responsible for services rendered.
  2. For your benefit, we will collect just your co-pay at the time of service and submit the claim to your insurance. You are responsible for paying the full amount determined by your insurance company once they have paid your claim. To deliver a seamless experience, we ask that you put a credit, debit, or HSA card on file in the event that there is a balance owed after the claim goes through insurance. After your insurance has processed the claim and issued an EOB, any remaining balance that is deemed patient responsibility (ie: deductible, co-insurance, etc.) will be charged to your card on file (not to exceed $1,500 max. annually).
  3. All credit card/debit card information will remain absolutely confidential, encrypted, and securely stored by Elavon for 365 days, unless you call to change the card on file.
  4. We will attempt to contact you via email prior to debiting your card, and at that time, you will have the option to submit payment in a different form, if you choose. You can reach out to our billing team at 303-558-0504 with any questions. If your insurance pays the claim in full, your account will not be charged and you will not receive an invoice.
  5. It is your responsibility to provide us with your most current billing information, all available telephone numbers and any other important contact information. If your address or contact information changes, it is your responsibility to contact us with the updated information.
  6. If a statement is received, payment in full is due upon receipt of the statement. Patient balances not paid in full within 30 days of the statement issue date are deemed past due. Past due accounts may be referred to a professional collection agency and/or attorney for further collection activity.
  7. If you are not able to pay the balance due in full, you must contact our billing office to discuss a payment schedule. If you fail to make payments as agreed upon, your account may be referred to a professional collection agency and/or attorney. You will be responsible for all collection costs incurred, including attorney’s fees and court cost if applicable.

Advanced Urgent Care & Occupational Medicine submits claims to insurance carriers as a convenience to our patients. You are authorizing benefits to be assigned to Advanced Urgent Care for healthcare services provided to you by Provider.

What is the credit card on file policy?

If you are using insurance for your urgent care visit, we ask that you put a credit, debit, or HSA card on file at the time of your visit. We will collect just your co-pay and submit the claim to your insurance. If upon receipt of an Explanation of Benefits (EOB) from your insurance carrier there is still a balance owed, you are responsible for payment. Therefore, we request authorization to charge a major credit/debit card for this unpaid amount.

What is your payment policy for motor vehicle accident visits?

We treat minor injuries from motor vehicle accidents. There are two options for payment:

  • You can pay for the visit upfront with our self-pay pricing and then you can submit a claim with your car insurance company
  • Or, Guardian Medical will facilitate payment with your auto insurance company for your initial visit with us and any follow-up visits with them. Call Guardian Medical at 303-222-0363 to learn more

Car insurance, rather than health insurance, is used for the payment of treatment related to auto accident injuries. If your visit to our clinic is related to an injury sustained in a motor vehicle accident, a claim will need to be submitted to your car insurance company. Our staff will provide you with all necessary paperwork and answer any questions you may have. You also have the option of scheduling a follow-up visit with Guardian Medical. They will facilitate payment for your initial visit and all follow-up visits at their facilities.

How do I pay my bill online?

If using a desktop computer, please click the "Pay Your Bill" button at the top of our website. If using your phone, the "Pay Your Bill" option is in the navigation menu.

Do you take Care Credit?

Yes, we accept Care Credit as a form of payment. Make sure to go to your Care Credit portal and select Advanced Urgent Care as a payer.

Does insurance cover rapid COVID testing?

Rapid Testing LLC facilitates rapid tests for medical conditions like COVID-19 and strep throat without needing a provider visit. Rapid Testing LLC is not a medical facility. However, they partner with medical clinics to offer convenient curbside testing at an affordable rate.

Patients can submit their Rapid Testing LLC invoices for COVID testing to their insurance company to see whether they can get reimbursed. We also accept HSA cards for all Rapid Testing LLC services.

How much does an urgent care visit cost?

In general an urgent care visit costs between $150 and $250.

What is the cost without insurance?

We offer a discount to patients who are uninsured and paying at the time of service. We have two pricing tiers: $165 or $250. Keep in mind that we also accept Care Credit as a form of payment.

Do urgent cares accept insurance? Which insurances do you take?

Yes, Advanced Urgent Care accepts health insurance, including Medicare and Medicaid. Some of the major insurance companies we contract with, include (but are not limited to): Aetna, Humana, TRICARE, Anthem Blue Cross Blue Shield (BCBS), Cigna, Kaiser PPO, and UnitedHealthcare (UHC). 

Not all plans are are considered in-network, however, we do submit all insurance claims on behalf of the patient — regardless of the insurance company. Please call your insurance carrier before your visit to determine your own financial responsibility. 

We offer discounted pricing to patients who are uninsured or have out-of-state Medicaid.

Do you have to pay upfront at urgent care?

Yes, we typically require payment at the time of service, but see below for different scenarios.

If you have insurance, you'll just need to pay your copay at the time of service. If you do not have a copay, or have met your plan's yearly deductible, then you are not required to pay at the time of service. Keep in mind, once your billing claim has gone through insurance you may have a remaining balance. The balance will be paid with the credit card on file or you will be issued a bill.

If you are uninsured, an urgent care visit is $165 or $250 based on the services you need. Payment is due at the time of service.

If you are here for a sports physical, DOT physical, non-DOT physical, or TB test, you'll pay at your visit for these services.

If you are here for an employer-paid service, you will not need to pay at the time of service. However, we will need a completed employer authorization form so we can bill your employer.

Do you need health insurance for urgent care?

You can be seen at the urgent care without health insurance.
