How to Access and Print Your COVID Results

Your COVID results letter is available in our Patient Portal. Please make sure you have a Patient Portal account. You’ll receive an email from our team at the time of your test to sign-up. Or, if you’ve already been seen at Advanced Urgent Care & Occupational Medicine you can sign up for an account. Click here to create a Patient Portal account. You can also access the Patient Portal by clicking on the “Patient Portal” button on the top of our website.

Directions for Accessing and Printing COVID Results Letter on Your Phone

1. Click here to login to the Patient Portal or click on the “Patient Portal” button on the top of the Advanced Urgent Care & Occupational Medicine website.

2. Once the page loads, click on the blue button labeled “Log in with athenahealth.” A new screen will load and that is when you’ll enter your login information.

3. Next, you’ll need to view the full version of the Patient Portal website. Scroll down to the bottom of the page and click on “Full Site.”

4. Now click on “My health” at the top of the screen.

5. Then select “Medical Forms” below the “My Health” heading. Next, you’ll click on the “Rapid COVID Results Form” under the “Signed Documents From Your Providers” heading. A PDF will automatically start downloading.

1. Click here to login to the Patient Portal or click on the “Patient Portal” button on the top of the Advanced Urgent Care & Occupational Medicine website.

2. After logging into the Patient Portal, click on “My health” in the column on the left side of the screen.

3. Next, click on the “Medical Forms” tab near the right side of the screen.

4. Then, under “Signed Documents from Your Providers” you’ll click on “Rapid COVID Results Form.” A PDF will automatically start downloading.
