Kids registering to participate in school sports or going to camp are typically required to have a physical examination and a signed release from a provider. Make to sure to bring the CHSAA Pre-Participation Physical Evaluation form to your visit!

School physicals can be completed at any of our Denver metro clinics. These are for college-aged athletes. Please contact a clinic if you need help registering for this, or simply walk-in.
The typical physical exam includes a height and weight measurement, blood pressure check, and a look at the eyes, ears, and throat. The provider will likely listen to the heart and lungs for healthy patterns and feel the abdomen for any abnormalities.
