Personal Data

Advanced Urgent Care & Occupational Medicine

During the electronic registration process at every Advanced Urgent Care & Occupational Medicine clinic, we ask whether you would like to receive email and SMS communication updates about our business. We also ask attendees at events and visitors to our website.

When you elect to receive this communication, we add your contact information to our marketing communication platform (Constant Contact). You can choose to receive SMS communication, email communication, or both.

If you give us permission to email or text you marketing information, we then store the following personal information on our marketing communication platform:

  • First name & Last name
  • Email address
  • Mobile phone number
  • Language
  • Birthday
  • Patient ID number
  • Zip code

Rapid Testing LLC

When you schedule a visit at one of our Rapid Testing LLC sites, we collect and store your personal data on our Rapid Testing scheduling platform (10to8) and on our marketing platform (Constant Contact).

The personal data stored on 10to8 includes:

  • First name & Last name
  • Email address
  • Mobile phone number
  • Appointment type
  • Language
  • Birthday
  • Zip code

If you give us permission to email or text you, we then store the following personal data on our marketing platform (Constant Contact).

The personal data stored on Constant Contact includes:

  • First name & last name
  • Email address
  • Mobile phone number
  • Birthday
  • Patient ID number
  • Zip code

 

How We Use Your Personal Data

We communicate with our patients by sending both email and SMS messages approximately 3x/month. We never sell your contact information or share it outside of our organization.

We may use Personal Data for the following purposes:

  • To contact you by email, telephone calls, or SMS, or other equivalent forms of electronic communication.
  • To provide you with news, special offers, and general information about services and events which we offer.
  • For other purposes, such as data analysis, identifying patient trends, determining the effectiveness of our promotional campaigns, and to evaluate and improve our services, marketing, and your experience.

You can unsubscribe from our communication anytime by selecting the “unsubscribe” link in the footer of our email or replying “STOP” to a text message. When you unsubscribe from any of these communication methods, your contact information (personal data) is still stored in our marketing platform, so that the system knows not to contact you.

We do not charge for SMS messages but you are responsible for any message and data rates imposed by your mobile provider, as standard data and message rates may apply.

How We Use SMS for Non-Marketing Purposes

Non-marketing SMS content is related to your health management and visit follow-up. Messages may include appointment confirmations, visit reminders, lab results, x-ray results, or payment requests.

We may send SMS messages through our affiliate healthcare platforms, including ClockwiseMD (ExperityHealth), 10to8, and Upfront Healthcare. Message frequency will vary. You agree that we may send you messages regarding your visit and treatment, and that such messages and/or calls may be made or placed using different telephone numbers or short codes.

We do not charge for SMS messages, but you are responsible for any message and data rates imposed by your mobile provider, as standard data and message rates may apply.

Deleting Your Personal Data

If you ever want to update, amend, or delete your information from our marketing platform, please contact us.

Please allow 30 days for the requested updates. Within those 30 days, we will contact you and confirm the removal of your personal information from our marketing platform.

Personal Data for Marketing vs Protected Health Information

The personal data we store for marketing and communications purposes is protected differently than the personal health information stored as part of your medical records. Your personal health information is stored within our Electronic Medical Records (EMR) and is protected under HIPAA.

Usage Data

Usage Data may include information such as your device’s Internet Protocol address (e.g. IP address), browser type, browser version, the pages of our website that you visit, the time and date of your visit, the time spent on those pages, unique device identifiers, and other diagnostic data.

When you use a mobile device, we may collect certain information automatically, including, but not limited to, the type of mobile device you use, your mobile device unique ID, the IP address of your mobile device, your mobile operating system, the type of mobile Internet browser you use, unique device identifiers, and other diagnostic data.

We may also collect information that your browser sends whenever you visit our website or when accessing our business through a mobile device.

Tracking Technologies and Cookies

A cookie is a small file placed on your device. You can instruct your browser to refuse all cookies or to indicate when a cookie is being sent. However, if you do not accept cookies, you may not be able to use some parts of our service.

We may update our Privacy Policy from time to time. We will notify you of any changes by posting the new Privacy Policy on this page.
